Do I Really Need a Facelift? My Consultation & Cost Breakdown!

We all have unique anatomy and facial concerns as we age. However, a common theme for many of us in our 40s is hollowing under the eyes as our cheek fat pads descend due to gravity. This descent becomes apparent when we notice our nasolabial folds (the parentheses around our mouths) deepen, our cheeks lose their plumpness, and our necks begin to sag.

What’s Dr. Scott Green’s opinion of my face and what I need?

I met with Dr. Scott Green on October 23, 2024. The following is his assessment of my face and neck.

NECK

First, he addressed my neck. You can see loose skin from weight loss and age. I’m currently 42 years old, and during our forties, we notice our necks descending, creating that “gobble” look.

As we age, our platysmal bands stretch, creating vertical lines on our necks. These bands, which extend from our mid-face down to our chests, weaken our jawlines as they loosen over time.

Dr. Green explained where these incisions for the neck lift are made and how they are hidden in the hair line and in the ear.

EYES

Under eye crepey skin.

To address the hollowness under my eyes and the drooping upper lids, Dr. Green recommended a combined lower and upper blepharoplasty. This procedure can effectively correct these eye-area concerns.

Dr. Green is great at explaining the blepharoplasty incisions and resulting scars.

LIPS

I’ve seen impressive lip lifts that can shorten the philtrum and enhance the upper lip. This procedure lifts and turns out the top part of the lip, creating a more youthful appearance. I wanted to see if Dr. Green thought this was a possibility for me.

After examining my lips, Dr. Green advised against a lip lift for me. He explained that lip lifts, like most elective procedures, require careful consideration. In his opinion, the resulting scar isn’t worthwhile unless the philtrum is extremely long and the top lip is barely visible.

In the example above, he demonstrated how this patient’s lips were enhanced by lifting the face rather than performing a lip lift.

I also inquired about the corner mouth lift surgery. Again, he cautioned that this procedure can be tricky, as some patients end up with a “joker-like” smile.

FACE

Dr. Green explained that lifting the mid-face would provide the best results for me. In a mid-facelift procedure, the cheek and soft tissue that have dropped due to gravity can be pulled up vertically and anchored in place. This ensures longer-lasting results compared to a deep plane facelift. In the video, Dr. Green explains the differences between the deep plane facelift and the mid-facelift—I think it’s excellent insight. However, he noted that for patients with very thin, compromised skin or smokers, a deep plane facelift might be best due to blood flow considerations.

You can see the cheek drop, which hollows out my under eyes and creates deeper nasolabial folds, this also leads to the jowls we begin to notice.

The mid-facelift and anchoring up into your skull. This keeps soft tissue in place longer.

The mid-facelift utilizes small incisions in the scalp and eye to lift this fallen tissue.

FACIAL FAT GRAFTING

When assessing my face, Dr. Green brought up facial fat grafting and explained how it could drastically improve and restore a more youthful appearance.

During his explanation of facial fat grafting, Dr. Green revealed that the best fat for facial transfer comes from our knees and thighs. He had performed my mommy makeover surgery in February 2023, which included liposuction on these areas. Given my thin frame, he noted we’d need to carefully search for fat, focusing on my legs since leg fat typically lasts longer when transferred to the face.

Legs are the best place to take fat for facial fat grafting.
