Does Microneedling Have to Hurt?

One of the questions I receive most often is:

“Do you numb before microneedling?”

The answer depends on the depth of treatment and your comfort level.

For lighter microneedling sessions (around 0.5–1.0 mm), I often don’t numb because I’m comfortable with the sensation after years of treatments. However, when I’m planning a deeper collagen induction session—typically 1.5–2.0 mm in selected areas—I prefer to numb my face beforehand to make the treatment much more comfortable.

Today I’m sharing my complete pre-microneedling numbing routine, exactly as I do it before a deeper facial treatment.

Why I Numb Before Deeper Microneedling

The goal of numbing isn’t simply to eliminate discomfort.

It’s to allow me to comfortably perform a thorough treatment without rushing or avoiding sensitive areas. When I’m comfortable, I can focus on technique rather than pain.

Step 1: Start With Clean Skin

Every microneedling treatment begins with freshly cleansed skin.

Removing makeup, sunscreen, oil, and debris creates a clean surface before applying numbing cream.

Step 2: Pre-Treat With a Dermaroller

Before applying numbing cream, I lightly roll my face with a 1.0 mm dermaroller.

Some people prefer a 0.5 mm roller, which can also work well.

I use gentle passes:

  • Vertical
  • Horizontal
  • Multiple directions over areas of concern

This helps prepare the skin before applying the numbing cream.

I also spend extra time on areas I specifically want to treat, like an old chickenpox scar on my forehead.

Step 3: Apply the Numbing Cream

After rolling, I generously apply MuchCaine 5% over the treatment areas.

I use a silicone spatula to spread an even layer across my face.

Sensitive areas like:

  • cheeks
  • under the eyes
  • jawline

receive a generous application because they tend to feel more sensitive during treatment.

If I’m treating my nose, I’ll also apply cream there after lightly rolling the area.

Step 4: Let It Sit

I typically allow the numbing cream to work for:

30–40 minutes

before beginning microneedling.

This provides enough time for the cream to take effect before treatment begins.

Should You Use Plastic Wrap?

Sometimes.

Plastic wrap creates an occlusive barrier that may help the numbing cream work more efficiently.

If I’m short on time, I’ll cover the cream with plastic wrap.

If I have a full 30–40 minutes available—as I did during this session—I usually skip that step.

Preparing for Microneedling

Once the numbing time is complete:

  • Remove the numbing cream.
  • Thoroughly cleanse the skin.
  • Prepare your microneedling device.

For this treatment I used:

Because I planned a deeper session, numbing made the experience significantly more comfortable.

After Microneedling

After completing my treatment, my skin felt very raw—more intense than a typical sunburn.

To calm my skin, I applied a soothing mask and used my Lumara LED mask during my live session.

I also chose acetaminophen (Tylenol) for discomfort rather than ibuprofen because I wanted to avoid reducing the normal inflammatory response that’s part of the skin’s healing process after microneedling.

Final Thoughts

Numbing isn’t always necessary.

For shallower treatments, many experienced users are comfortable without it.

However, when I plan a deeper collagen-induction session, taking 30–40 minutes beforehand to numb my face makes the experience much more manageable.

Everyone’s comfort level is different, so choose the approach that works best for your treatment goals and experience.

Frequently Asked Questions

Do you always numb before microneedling?

No. For treatments around 0.5–1.0 mm, I often don’t numb. For deeper sessions around 1.5–2.0 mm, I usually choose to numb for added comfort.

How long should numbing cream stay on before microneedling?

I generally allow 30–40 minutes for the numbing cream to work before removing it and beginning treatment.

Why do you use a dermaroller before applying numbing cream?

I lightly roll the treatment areas before applying numbing cream as part of my personal routine before deeper microneedling sessions.

Should I use plastic wrap over numbing cream?

Plastic wrap can help create an occlusive barrier and may improve penetration if you’re short on time. If I have a full 30–40 minutes available, I often skip it.

What microneedling device did you use?

For this treatment I used the Dr. Pen M9 together with Dermaheal HSR.

What depth do you microneedle?

Treatment depth varies depending on the area being treated and individual goals. In this session, I planned to work deeper than my usual maintenance treatments.

What does microneedling feel like afterward?

Immediately afterward, my skin felt more intense than a sunburn and very worked over. That sensation gradually improves as the skin begins healing.

What do you do after microneedling?

I focus on soothing my skin by applying a calming mask, using LED light therapy, and following an appropriate post-treatment skincare routine.
